RMS Nurse Office / Attendance

Please call the school attendance line whenever your child will be out of school to make the school aware that your child is absent. 

Within ten (10) days of an "incident of absence," a written note must be sent to your school office or an Online Reason for Absence Note completed.

Additional information may be obtained from the school nurse:

• Fever 100° or Above - keep your child home until their fever has been gone for 24 hours without the use of fever-reducing medications
• Vomiting and/or Diarrhea - keep them home for 24 hours after the last incident
• Strep Throat: 24 hours after the first dose of antibiotics; fever-free for 24 hours, and feeling well enough to participate in school.
• Conjunctivitis (aka Pink Eye): Antibiotic treatment for 24 hours or three doses and no drainage from the eye.
• Students with the following illnesses are not permitted to ride the bus or return to school until cleared by the school nurse:
  • Lice: Appropriate treatment must be completed. A no-nit policy is enforced. Call the school nurse for the Region 15 guidelines before returning to school.
  • Impetigo: Treatment for 24 hours with the affected area covered.
  • Varicella (aka Chicken Pox): Until the last eruption has crusted over.

COVID Information:

If you have COVID-19, you should remain home until you have been fever-free without medication for 24 hours or your symptoms have been improving for 24 hours.  We encourage masking and good hygiene.  COVID-19 tests are available in the school nurse's office. Please contact your school nurse if you need test kits.

Each student must submit a Health Assessment Record before initial enrollment in the Region 15 Public Schools and shall have a health assessment completed at least once in each three year interval: in Pre K, K, 6th and 10.

Please contact your child's school nurse regarding any serious injuries or illness.

If your child has an injury or illness that prevents full participation in physical education/recess, communication from your medical provider is required specifying activities that must be modified or avoided. 

A Physician's note/order is required for any type of restriction due to injuries.

Any student in need of a medication on a field trip must notify the nurse's office one week prior to the field trip. A doctor's order is mandatory for prescription or over the counter medications. 

All vision, hearing and postural screenings for the mandated Kindergarten, 6th, and 10th grade physicals should be performed and recorded by your physician on the Student Health Assessment Record.

In school, vision screenings are done in Grades K, 1, 3, 4, 5, 6 &10; Hearing is tested in Grades K, 1, 3, 4, 5, 6 & 10; and Postural Screenings are done for females in Grades 5 & 7 and males in Grade 9.

A referral form will be sent home if your child does not pass the screening test. Please follow up with the referral and submit the results from your medical provider to the Health Office.

Tuberculosis Screen: Students moving into Region 15 from ANY High Risk country need a tuberculosis screening with results interpreted/documented by their physician's office prior to entering the school district. Students traveling outside of the US to high risk areas (even for vacation) will be required to have a TB test within 6-8 weeks after return to the U.S. The date of the PPD appointment must be given to the nurse upon return. The PPD must be read 48-72 hours after it is planted and printed results must be given to the school nurse. Additional screening may be required according to the Tuberculosis Policy
